  Preparation for Your Appointment
  1. New Patients - Welcome to the practice! We at the UroHealth Institute and Urologic Surgery Associates all look forward to taking care of you and your health care needs. We want to provide a comprehensive approach to taking care of your urologic issues as well as you, the patient. Your experience should be as pleasant as possible. We want to treat each and every patient as an individual with their psychological and healthcare needs being held paramount. To provide you with the most streamlined service and to serve you in the fastest most efficient manner a few things in preparation before your appointments will avoid any delay.

    1. Prior to your appointment - Over the phone you will need to provide your insurance information, a brief explanation of why you are visiting us (if you do not want to give a reason simply state this), and many insurance providers require you have a referral from a primary care physician before seeing a specialist at Urologic Surgery Associates. All co-pays are due at the time of the appointment. We take cash, personal checks and credit cards (Visa, MasterCard, and Discover). Be sure to bring your insurance card with you each time you come in for your appointment. We also get a copy of your driver's license for billing and contact information. By providing these few items will assure you the fastest service possible. Call ahead to our staff with any questions and they can assist you as they are very familiar with most situations (309-888-4000). Insurance specialists are available to answer questions before your appointment or at any time during your experience with us. If for any reason you need to cancel please call to let us know and reschedule if appropriate, no fees are charged.

    2. The day of your appointment - When you arrive we will have you fill out various forms. These may be downloaded below and filled out before you arrive at your appointment but be sure not to forget to bring them.

      1. Welcome letter
      2. Patient Information sheet
      3. Medical History Form - 2 sides
      4. Informed consent
      5. Financial Information
      6. Privacy Notice (Health Insurance Portability and Accountability Act - HIPAA form) - 2 pages each double sided
      7. Acknowledgment of receipt of privacy notice

    3. Your First Visit - In order to accomplish our goal of providing optimal health care within a reasonable amount of time, your first visit at our office will require, at the minimum, a urine test, medical history and physical exam. Additional tests will be discussed and scheduled appropriately as needed. We ask that your please have the following available during your first visit:

      1. Insurance / Medical cards - We need the name, date of birth and social security number of the primary and secondary insurance card holders. If you have more than two insurance plans, please include information on all your plans.
      2. Complete list of medications with their dosages.
      3. Reports of previous laboratory tests and/or x-rays, if possible.
      4. Patient information sheet. This is available for download above or will be provided to you at the time of your visit. If you would like to call the office before your visit we can also mail you the forms to be filled out before your arrival. The forms need to be completely filled out including all insurance information.
      5. Driver's license or other picture ID - for identification and contact information
      6. If your insurance is with Medicaid/ Illinois Public Aid strict guidelines have been set by the state to delver your medical care. In compliance with Illinois regulations, patients with Illinois Public Aid coverage must present a current card at the time of service. If you do not have a current card with you at the time of your appointment, your appointment will be rescheduled.

  2. Return Patients - Welcome back. As you have already seen us at Urologic Surgery Associates your information is in our system. PLEASE keep us updated to any changes in your health, recent procedures, new medicines, changes in address, changes in your insurance, or any other changes you feel should be updated. If your insurance requires referrals before your appointment please have these arranged. As always bring your insurance card and co-pay with you. Our friendly and experienced staff would be happy to assist you with any questions along the way. As you might imagine they have done this before and can likely help with any of your questions. As usual if you need to cancel for any reason call as far in advance as possible, no fees are charged.

  3. Procedure Visits - If you having a surgical procedure performed at the UroHealth Institute you will be given a time to show up by the surgical scheduler. She will provide you with detailed instructions on how to prepare for the surgery. You will be contacted the day before your surgery to remind you of when to arrive at the UroHealth Institute. Below is a general list that applies to most surgeries, BUT, for specific instructions for selected surgeries please see the special test instructions section

    1. when to stop eating,
    2. what medicines to stop taking before surgery,
    3. what medicines to take the morning of surgery,
    4. Pre-operative testing etc.

  4. Nurse Visits - Often times the physicians at Urologic Surgery Associates will order laboratory tests, nurse procedures, or x-rays to be performed. These will be arranged through the scheduling office. Depending on what is being done, you will be scheduled a time to come in or you will be told to drop by during our business hours at your convenience. You will at that time see either a nurse or technician who will perform what has been ordered. In most cases you will not see your physician at the time of the visit as many results are not immediately available. Your results will be discussed at your next doctor's visit or you will be contacted by phone as to your results and any further instructions. If you have not heard from us by 2 weeks after your tests have been performed please call to ask for results. As with all visits, be sure to bring your insurance cards and co-pays. Additionally, PLEASE keep us updated to any changes in your health, recent procedures, new medicines, changes in address, changes in your insurance, or any other changes you feel should be updated.

  5. Phone Nurse - A urologic phone nurse is available to answer medical questions during our business hours Monday through Friday 8:30 a.m. to 4:30 p.m. All of our nurses have extensive experience in urology and if they are unable to answer your questions they will contact your physician. If she is on the line with another patient you will be forwarded to voicemail which is checked continually. In the event of a life threatening medical emergency call 911, otherwise the phone nurse will call you back as soon as possible. For medicine refills please see the Prescription Refills section.

  6. After Hours - For emergencies after hours, the answering service will contact the on call physician. In the event of a life threatening medical emergency call 911. To contact the answering service please call (309)-823-0310 on the weekends or after hours.

  7. Emergencies - In the event of a life threatening medical emergency call 911.
